Video & Photography Release Terms

I hereby grant and authorize Care Solace, Inc. (“Care Solace”) the right to take, edit, alter, copy, exhibit, publish, distribute and make use of any and all video, audio, photographs, or other media recordings of me during the 2023 Care Solace Mental Health Summit (the “Media”), to be used for the lawful business purpose of including Media in the content of Care Solace social media campaigns, and/or publishing and archiving Media consisting of full videos of panel discussions (the “Purpose”).

This authorization shall continue indefinitely unless I otherwise revoke this authorization in writing.
